I think your framing is to tight. Your camera seems to have been pointed too high. As far as lighting, a bit more contrast on the skin would be better.

Hmmm, I like it, the lighting works in my mind. The only thing I would change would have been to get both eyes in focus. Just a tad more DOF.

Lovely tones and contrast with the darker lines of the eyes and lips. Given that there is free space above the hairline, I think the uneasy crop through the lip unbalances the composition (vertically). The hair isn't contributing that much so maybe raise the lips a little higher and crop above the eyebrows (or include more hair).
