It is all done in photoshop. You can use adjustment layers or you can make it without.
Use Gradient map to convert image to monochrome - b&w and then boost the contrast until you satisfy with result and look. Boosting contrast will not work on all images to create such look. With adjustment layers - the same steps but then you work and manipulate layers.
